Hi Mine, I believe you're referring to this round:

In the next round, we’ll join both legs and continue to make the body.
Rnd 11: ch 4, sc in next 18 st on the first leg, sc in next 4 ch, sc in next 18 st on the second leg, sc in next 4 st on the other side of ch 4 [44]

Your crochet hook is currently in the second leg, you don't fasten off this round. You work 4 chains and then insert your crochet hook in the first leg. On this first leg you work 18 sc stitches. Then, insert your crochet hook in the chains you've worked and sc in all 4 chains. You then continue on the second leg where you started and crochet 18 sc on this leg. You finish the round by crocheting 4 sc in the other side of the chain.
