I think you're referring to this part?

Rnd 1 – 4: sc in all 27 st [27]
Rnd 5: ch 13. Place the stitch marker in the next stitch
you’ll make, as this is going to be the beginning of each
round from this point on. Crochet back on the chain,
inc in second ch from hook, sc in next 11 st, sc in the
stitch where the foundation chain starts, continue on
the neck, sc in next 27 st, continue on the other side of
the chain, sc in next 11 st, inc in last st [54]

So you've made the neck of your unicorn, which is 27 stitches in the round. Next, you want to continue crocheting to make the body, but the body is much wider. Therefor you work a chain of 13 ch and you crochet back on the chain following the instructions, alike you would do if you would make a starting chain. You then continue crocheting on the neck. When arriving at the starting point of the chain, you continue crocheting on the chain.

From now on, your body shape has the size of the neck + the additional strip you made. You continue crocheting around this shape.
